Introduction

In the event that the health authorities announce a new period of confinement due to the evolution of the health crisis caused by COVID-19, the teaching staff will promptly communicate how this may effect the teaching methodologies and activities as well as the assessment.


Justification.

The planning of nursing care for major changes in the health of people requires a solid foundation of general skills that will be repeated in learning the care plan for each pathology. Progressively discover and internalize these skills with a methodology that combines the Master Class for the more complex and novel issues, while the remaining topics are reserved for group tutoring, preparation and presentation of information, and the facilitation of the assimilation of skills.   

Descriptor.

General Clinical Nursing subject focuses on acquiring the knowledge of recognizing adult health disorders, manifestations and care, and applying the Nursing Care Process P.A.E (from the Spanish term Proceso de Atención de Enfermería).During the course of the subject students will preform the care procedures to treat and / or prevent arising problems in the patients and families through cooperative work with other team members and with the participation of the person receiving care and their families in the health and disease process. Highlights include elements that are the basis for other courses in areas such as Semiotics, Homeostasis and General Basic Principles of Clinical care.

Objectives

  1. Students will develop cognitive and psychomotor abilities, as well as relationships that enable them to integrate knowledge of basic general pathology in the adult, so to provide adequate nursing care skills, and promote the development of autonomy, health maintenance, disease prevention and welfare of the patient, family and community.
  2. Acquire the necessary skills to integrate theoretical knowledge in the daily practice of the nursing profession.
  3. Know general nursing care associated with more frequent semiology, the person in critical situations, surgical, chronic and oncological diseases, using the following methodology:
  4. Integrate specific knowledge of the disease based on the holistic concept of the individual.
  5. Remember the pathogenesis, epidemiology, pathophysiology of semiology scientifically based on nursing care.
  6. Knowing prognosis and possible complications.
  7. Study the Nursing Care Process (P.A.E) following: Obtaining correct information for the rigorous development of the nursing care plan. The specific assessment of the signs, symptoms and problems, using clinical judgment and establishing priorities. Care planning based on problems and nursing diagnoses.

Learning outcomes of the subject

  • Develop cognitive and psychomotor abilities and relationships which provide adequate nursing care to promote autonomy, health maintenance, disease prevention and the well - being of the patient, their family and community.
  • Recognize the most frequent symptomatology in major health disorders
  • Develop nursing care from the reflection of the pathogenesis, epidemiology, pathophysiology of semiology scientifically based on nursing care through the care plan which is based on problems and nursing diagnoses.
  • Know the main diagnostic test imaging (radiology, scanner, PET etc.) and analytical, its foundation and application, as well as the therapeutic application, especially in cardiovascular and oncology interventionism.

Syllabus

  1. Introduction to nursing care in medical-surgical diseases. The physical and chemical agents that cause disease. Thermoregulation.
  2. General Semiología. Nursing care in large clinical syndromes. Pain, fever, jaundice, nausea and vomiting, bleeding, dizziness, syncope, coma, dyspnea, oligoanuria.
  3. Disorders of electrolyte and acid-base balance. Nursing care in patients with disorders of fluid and electrolyte balance. Fundamental concepts of fluid and electrolyte balance and acid-base homeostasis. Nursing care in patients with Fluidotherapy. Volumetric disorders. Imbalances of the main intra and extra cellular ions. Nursing care in patients with disorders of acid-base balance.
  4. Infection. Nursing care to patients with infection.
  5. Shock, sepsis and multiorgan failure. Nursing care to patients with shock.
  6. Chronic disease and acute disease. Definition and causes of acute and chronic disease. Differential diagnosis strategies and treatment. Nursing care in patients with chronic diseases.
  7. Physiotherapy and rehabilitation. Concept. Methods. Main therapeutic indications. Nursing care in patients with self-care deficit, at risk or impaired physical mobility, skin integrity and altered pattern of elimination.
  8. General principles of medical and surgical oncology. Nursing care for cancer patients in relation to the   Cancer pathophysiology, epidemiology, prevention and early detection. Classification system: Tumors, Nodules and Metastasis. General aspects of cancer treatment.
  9. Clinical tests. Diagnostics Laboratory haematological, microbiological, immunological, biochemical and biological laboratory.   Normal values for the most common clinical analysis. Analytical requests in daily practice, and outpatient hospital clinics.  
  10. Diagnostic techniques. The clinical diagnostic process. Diagnostic radiology. Histopathological diagnosis. Endoscopy. Clinical monitoring. Special diagnostic techniques.  Preparation of the patient undergoing diagnostic tests.
  11. The surgical patient. Nursing care in the pre-operative period. Nursing care   Intraoperative: positioning the patient on the operating table. Basic rules of surgical asepsis. The operating room. Sterilization. Surgical instruments. Nursing care in the post-operative period. The post-operative period: post-operative care unit: Immediate interventions. post-operative complications. Nursing care in the surgical wound.  Surgical pathophysiology. The surgical wound.
  12. Intensive care. Nursing care to critically ill patients. Structure and function of intensive care.  Monitoring systems. Treatment systems.

BLENDED LEARNING METHODOLOGY

• The classes will be given in person in the UIC classroom and with approximately half of the students; and at the same time it will be broadcast live (streaming) for the students who are following it and participating at home; all with the purpose of maintaining safety distances in the classrooms. The distribution of days of attendance at the university will be indicated in a separate document.

• The teachers of the subject have programmed and coordinated different methodological systems according to the competences and content to be taught. In this sense, they range from theoretical classes with participation in debate and kahoot, to classes with presentation of the syllabus followed by individual student work to share and develop it together with the other students, both face-to-face and non-face-to-face. Some of these methodologies involve previously recorded training capsules for the study and individual or collective work of the student, to be later developed in class.

• Classes will not be recorded unless a student needs it for accredited reasons.19, in which case they will be given access to it.

 EVALUATION WHEN THE SUBJECT IS GIVEN WITH BLENDED LEARNING METHODOLOGY

• In relation to the previous year 19-20, the requirement to take a 5 in the final exam is eliminated to be able to make an average with the rest of the marks, since in this way the continuous evaluation is given greater strength, which will now have a value 50% to offset the percentages in the global grade. The 50% of continuous evaluation will be 10% for the self-employment questionnaires, 15% the works for class participation (questionnaires after class work 40%, quiz contest 48% and strategy 12%) and 25% the intermediate written test. We consider it important to keep the exams because it facilitates the retention of the subject, but by itself it does not represent the acquisition of the competences and we think that with the realization of a case that includes the most important questions of the subject a correct balance is achieved.

Regarding the second call, note:

• The percentages of the evaluation of the first call and the marks of the continuous evaluation will be maintained, with the exception that it will be possible to improve the mark of the autonomous questionnaires (up to 5). The final exam grade will be the one obtained in this second call. MH will not be given on 2nd call.
