Introduction

According to the National Committee of the Medical Specialty in Obstetrics and Gynecology at the moment has conceptualized as a specialty extensive and diverse content that achieves:

1) The physiology of pregnancy, childbirth and the postpartum period, as well as its condition and appropriate treatment.

2) The physiology and pathology, as well as its treatment, inherent bodies forming the female genital tract, including the breast.

3) The physiological and pathological phenomena of human reproduction and its treatment.

4) Preventive, mental and social necessarily relate to the previous paragraphs.

 

 

 

LANGUAGES OF THE SUBJECT: Teachers teach classes predominantly in Spanish, some in Catalan

It is essential that the student has approved the subjects:

Objectives

To know her anatomy and function of the female genital tract, including breast, identifying anatomic structures and knowing fundamental neuroendocrine functions.

Understanding the normal physiology of pregnancy and understand the systematic controls during pregnancy. Know how to apply and evaluate the results in the various phases of monitoring.

To know, know how to identify the main diseases associated with pregnancy or own and consider preventive measures, early diagnosis and appropriate treatment to the person affected.

Knowing the mechanism of normal labor and detect when it needs a more comprehensive control.

Detecting the normal portion, its causes, prevent or treat complications customizing needs.

Being able to properly collect medical information relevant to the pregnant, postpartum or midwife.

To know and understanding the etiology, symptoms, diagnosis, prevention and personalized treatment of benign gynecological diseases. Know the Information Pre and post surgery.  Assessing prognosis in chronic gynecological diseases.

Get the pathogenesis, prevention and management of malignant gynecological and breast. Analysis of prognostic factors and be able to explain them in a clear and understandable way.

GENERAL THEORETICAL FRAMEWORK:

Introduction to the subject.

Reminder of the most relevant aspects of obstetric-gynecological pathophysiology. Reproductive anatomy and physiology: ovulation and menstrual cycle. Development of women. Placental conception and development. Embryo and fetus. Terminology and own records.

Introduction to gynecological-obstetric practice. Pregnancy diagnosis.

Objective: It is necessary to know and understand the genital morphological and functional anatomy towards the correct application in clinical practice.

 

GYNECOLOGICAL ENDOCRINOLOGY AND HUMAN REPRODUCTION:

Unit 2. Gynecological problems in childhood and adolescence. Pubertal development. Menstrual cycle: onset and abnormalities. Abnormalities: precocious and delayed puberty. Other gynecological disorders in childhood and young women. Premenstrual syndrome.

Objective: The student must know the normal and pathological sexual development, as well as the gynecological alterations that occur in childhood and youth. They should be able to explain the pathophysiological mechanisms of menstrual disorders and other related endocrine disorders and make the differential diagnosis.

 

MC 1. Gynecological outpatient management 1: 2 h. Dr. Roca

Unit 3. Amenorrhea.

Objective: to get students to develop a diagnostic and therapeutic management scheme for menstrual cycle disorders and metrorrhagia.

 

LH 1. Examination of women in gynecology and breast. 2 h. Dr. Xiberta

Objectives: Ethical management of exploration. Bimanual examination. Cytology. Basic gynecological surgery. Students should be able to explain the history and physical examination of the woman, how to obtain cultures and cytologies. Make a list of problems. When caring for women, show a cooperative, impartial and supportive attitude. They must be aware that they must protect the interests of patients.

Unit 4. Hyperandrogenism

Objective: That the students develop a diagnostic model of the hormonal alterations that can cause hirsutism and virilization, especially those of gynecological interest, based on the mechanisms and functions of the hormones involved.

Objectives: Ethical management of the exploration of the pregnant woman and childbirth. Practice with simulation normal deliveries and abnormal presentations, episiotomy and repair.

 

BENIGN GYNECOLOGICAL PATHOLOGY: 4 hours CM + 6 hours CM

Unit 5. Genital tract infections. Vulvovaginitis. Sexually transmitted diseases. Pelvic inflammatory disease.

Objectives: To know the evaluation and treatment of vulvovaginitis. Diagnosis, evaluation and treatment of the most common sexually transmitted diseases.

Unit 6. Endometriosis. Dysmenorrhea Chronic pelvic pain. Pathogenesis, Pathology, signs and symptoms, differential diagnosis and treatment. Dysmenorrhea and chronic pelvic pain.

Objectives: They should be able to describe the theories of the pathogenesis of endometriosis, signs, symptoms, evaluation, diagnosis and treatment both medical and surgical. As well as making the differential diagnosis of dysmenorrhea and chronic pelvic pain, diagnostic and therapeutic strategies.

MC 3. Abdominal pain in gynecology. 2 h. Dr. Girvent

Objectives: Through several clinical cases with patients of different ages, to review the differential diagnosis of abdominal pain of gynecological origin, both acute and chronic. From dysmenorrhea in premenstrual syndrome to a brief description of vulvodynia. Influencing especially in the differential diagnosis providing tools to do it (diagnostic tables and diagrams).

Unit 7. Benign gynecological pathology: Benign tumors, cysts, atrophies and dystrophies and malformations of the genital tract. Benign vulvar disease. Uterine myoma. Benign ovarian cysts and tumors.

Objective: To know the most frequent benign pathology, how to diagnose it, make a differential diagnosis and decide the most convenient therapy and follow-up if necessary.

Unit 8. Premalignant alterations of the genital tract. Vulvar intraepithelial neoplasia. Enf. Paget of vulva. Neck intraepithelial neoplasia.

Objective: To know the premalignant pathology, the etiopathogenic mechanisms, the diagnosis and treatment and the adequate follow-up.

MC 4. Abdominal mass of gynecological origin. 2 h. Dr. Girvent

Objectives: With several cases of different ages, to review the differential diagnosis of abdominal masses of gynecological origin and the possibilities offered by imaging. Tumor markers and their usefulness in the suspicion of neoplasia. Treatment of gynecological masses. Genital tuberculosis.

Unit 11. Pelvic statics alterations. Urinary incontinence in women. Uterine prolapse, female urinary incontinence. Urinary infection in women, differential features.

Objectives: To know the mechanisms of pelvic statics, the pathogenesis of its alteration and the most appropriate treatment. Likewise, to know the mechanisms of urinary continence in women, the correct diagnosis and treatment of its alterations. Explain the etiopathogenic mechanisms of urinary tract infections, diagnosis and treatment of urinary tract infections in women.

Unit 12. Menopause. Menstruation and menopause. Signs and symptoms of menopause. Premature ovarian failure. Treatment.

Objective: to understand the evolution of menstruation during a woman's life, its premature absence and its treatment and menopause as one more vital status.

Unit 13. Human sexuality. Contraception. Characteristics of human sexuality. Sexual response. Sexual dysfunction Hormonal contraception, barrier contraception, intrauterine and postcoital. Natural planning. Ineffective methods. Sterilization.

Objective: They should know and understand how human sexuality and its dysfunctions are manifested. Know the contraceptive means with their contraindications and the definitive sterilization techniques.

Unit 14. Infertility and Sterility. Etiology. Evaluation. Male sterility. Idiopathic sterility. Sterility treatments.

Objective: to know the causes of infertility in both women and men, how to make the basic diagnosis and treatments of infertility.

Pathology of the vulva, vagina and cervix. Vulva: vulvar dystrophies, vulvar cancer. Vagina: infectious pathology. Cervix: preneoplastic lesions, HPV infections, cervical cancer. Role of cytology.

Pathology of the uterine body. Endometrial hyperplasia and cancer. Myometrial pathology: leiomyomas. Trophoblast disease.

Fallopian tube and ovary pathology. Infectious pathology of the fallopian tube. Ovarian tumors.

Objectives: To review postoperative complications in gynecology, especially related to the type of surgery: vaginal, laparoscopic or open. Complications in vulvo-vaginal surgery and those of lymphadenectomies are also reviewed. It will be used to review surgeries and indications.

Unit 16. Benign alterations of the breast. Anatomy, evaluation of signs and symptoms, diagnostic techniques. Mastalgia, galactorrhea, benign tumors.

Objective: To know the diagnostic techniques of the breast and its application in the benign pathology of the breast, differential diagnosis and treatment.

Unit 17. Breast cancer. Risk factors, risk assessment, histological types and staging, treatment, prognostic factors

Objectives: To know the etiopathogenic mechanisms, diagnosis, staging, treatment and prognostic factors and follow-up of neoplastic changes in the breast.

 

OBSTETRICS FUNDAMENTALS:

MASTER CLASS 13

NORMAL GESTATION: 2 h. Dr. Roca

Unit 18. Anatomical and physiological changes in pregnancy. Hormonal changes, changes in the genital tract, cardiovascular system changes. Respiratory, alimentary, renal, immune system. Increased weight.

Objective: the student must know and understand the anatomical-physiological alterations that pregnancy entails. The physiology of the placenta and fetus, in order to distinguish between normal and pathological processes.

Unit 19. Antenatal care. Prenatal diagnosis. Preconception care, physical and psychological preparation. Prenatal screening. Prenatal visits. Preparation for breastfeeding. Gestation in limit ages. Antenatal basic information. Minor complications during pregnancy.

Objectives: to recognize the relationship between good health before and during pregnancy and the results in fetal-maternal morbidity and mortality. Know the feto-maternal control during pregnancy to identify early alterations in the course of pregnancy to allow an effective intervention. Prenatal diagnosis and genetic counseling.

 

MASTER CLASS 14. 2 h. Dr. Bataller

Unit 20. Complications of the initial pregnancy: abortion and ectopic pregnancy.

Objective: To understand that it is an ectopic pregnancy, its early diagnosis for the maintenance of fertility and the reduction of morbidity and mortality. Being able to define the types of miscarriage, explain the diagnosis and treatment include the differential of first trimester hemorrhage. Likewise, they must know the legal criteria of IVE in Spain. Determine and treat fetal death at each of the trimesters without forgetting support for parents.

Unit 21. Complications in late pregnancy: antepartum hemorrhage. Placenta previa, placenta accreta, placental abruption. Trophoblast disease.

Objective: They should be able to describe the causes of third trimester bleeding, evaluate them and describe the most appropriate treatments. Also, expose the maternal and fetal effects of these alterations. Also the treatment of acute bleeding and the use of blood products.

Objective: to develop the development model of the human embryo and its characteristics based on the previous knowledge of medical embryology.

Unit 22. Childbirth and normal puerperium. Part canal, fetal head and fontanelles. The motor of labor: myometrium (uterine contractions), pain, expulsive. Normal delivery: control of the phases of labor, active management, elective cessation. False labor, Duration of labor. First, second and third stages of labor. The mechanism of labor. Placental abruption. Perineal inspection and repair. Analgesia and anesthesia in childbirth. Puerperium and lactation.

Objective: to understand and be able to describe the normal processes of childbirth, its physio-pathogenic bases and the care of normal delivery and the normal puerperium.

PUERPERIUM AND OPERATORY OBSTETRIC COMPLICATIONS.

Unit 23. The complicated puerperium: postpartum hemorrhage, puerperal infection, thromboembolism, postpartum psychiatric problems.

Objective: They should be able to explain the risk factors, differential diagnosis and treatment of puerperal hemorrhage, puerperal infection, thromboembolism, and the psychological alterations that may appear during the puerperium.

Unit 24. Obstetric surgery: Induction of labor, management of labor with drugs, instrumentation of labor, cesarean section, version, cerclage, uterine emptying. Fetal surgery.

Objective: To know the obstetric surgery techniques, their indications and the complications of their use.

 

MEDICAL-SURGICAL COMPLICATIONS OF GESTATION:

MASTER CLASS 17. Dra Roca

Unit 25. Medical complications: cardiovascular, venous thromboembolism, antiphospholipid syndrome, respiratory: asthma, TB, hematological: anemia, thalassemia and hemoglobinopathies, isoimmunization, neurological: epilepsy, headaches, facial paresis, carpal tunnel syndrome and digestive: intrahepatic cholestasis, inflammatory bowel disease, appendicitis.

Objective: They should be able to expose the diagnosis during pregnancy, the impact on the mother-baby binomial and the initial treatment during pregnancy.

 

MASTER CLASS 18. Dra Roca

Unit 26. Endocrine disorders in pregnancy: diabetes mellitus, thyroid disease, pituitary and adrenal disease.

Objective: They should be able to expose the diagnosis during pregnancy, the impact on the mother-baby binomial and the initial treatment during pregnancy.

 

GESTATION COMPLICATIONS: FETUS I PART.

MASTER CLASS 19. 2 h. Dr. Xiberta

Unit 27. Placenta and adnexal diseases: Trophoblastic disease. Amniotic fluid alterations, umbilical cord abnormalities. Hypertension and pregnancy.

Objective: You should know the most frequent alterations, their diagnosis, the effects on the mother-baby and their treatment. In the case of trophoblastic disease, diagnosis, classification, initial treatment, prognostic factors, and adequate follow-up.

Hypertensive disorders in pregnancy: hypertension, preeclampsia, eclampsia, chronic hypertension.

Objective: you should be able to define and classify hypertensive disease in pregnancy, preeclampsia, eclampsia and HELLP syndrome, expose the physiopathogenesis, diagnosis, evaluation and treatment, including in childbirth and understand the effects on mother and fetus.

 

MASTER CLASS 20. 2 h. Dr. Xiberta

Unit 28. Multiple gestation. Abnormal presentations of the fetus and its management.

Objective: Diagnosis, evaluation, follow-up and control of the prenatal mother-twins and twin-twin pairs, during childbirth and the puerperium. Twins with abnormal growth or other related to high risk.

Know the abnormal presentations, how they are diagnosed and treated. Basic pathophysiology and fetal monitoring.

Review the topic of twins and growth and their alterations, review growth retardation, the influence of twins on prematurity, and diagnose alterations in the course of these pregnancies. Treatments available.

Unit 29. The high-risk fetus: risk pregnancy monitoring. Fetal suffering. Small fetus for gestational age.

Objective: Know how to recognize high-risk pregnancy, diagnose it, evaluate the fetal state and know the alarm signals. Know the normal fetal growth and recognize its alteration, whether with growth retardation or macrosomia, how to evaluate and treat it.

 

MASTER CLASS 21. 2 h. Dr. Xiberta

Unit 30. Alterations in the duration of gestation: APP, RPM, prolonged gestation.

Objective: to identify the normal duration, understand the evaluation when it is prolonged and expose the maternal-fetal complications of prolonged gestation. Likewise, it is necessary to know the most frequent mechanisms of shortening, the threat of pre-term labor and premature rupture of membranes as serious alterations in the normal evolution of pregnancy, their etiopathogenic mechanisms, the diagnosis and the most appropriate treatment.

Unit 31. Abnormal part (dystocia) and prolonged labor: presentation of the fetus, feto-pelvic disproportion, abnormalities in uterine mechanics, precipitated part, tetanitzat uterus, shoulder dystocia, prolonged labor, obstructed labor, uterine rupture. Pelvic abnormalities. Intrapartum sacrament: vasa previa.

Objective: To know adequately the alterations in childbirth, the adequate monitoring of it, the most adequate diagnosis and treatment.

 

MEDICAL COMPLICATIONS OF PREGNANCY. INFECTIONS.

MC 11. Medical complications and pregnancy. Infections and pregnancy. 2 h. Dr. Roca

Objectives: They should identify each of the infections during pregnancy, the impact of the infection on the mother-baby binomial and the most appropriate treatment for each of them.

MC 12. Other infections and pregnancy. Gestation and HIV. Gestational control and delivery. 2 h. Dra. Canto

Unit 32. Other infections during pregnancy: UTI, vaginitis, carrier β-hemolytic streptococcus, viral, tropical diseases (malaria, helminth infections).

Objectives: pregnancy and AIDS, prevention before pregnancy, control during pregnancy, treatment, part way. Taking the opportunity to review normal gestational control and especially with chronic viral infections: HIV, hepatitis, etc.

 

MC 13 (ABP). Prolongation of the second phase of labor or another phase related to labor. 2 h. Dra. Canto

Objective: to review the second phase of labor and its alterations, diagnosis, control and adequate treatment.

 

MC 14 (ABP). Obstetric outpatient management. 2 h. Dra. Canto

Detection of risk factors in pregnancy. Complications in normal delivery

Objective: to develop an effective risk factor detection scheme throughout pregnancy. Diagram of detection of the most frequent alterations in normal delivery.

 

MC 15. Puerperium complication: hemorrhage or infection. 2 h. Dr. Mazarico

Objectives: with several cases to review puerperal hemorrhage, its causes, early diagnosis and preventive and remedial treatment. Puerperal infection, etiology, diagnosis and treatments.

Teaching and learning activities

Before the beginning of the course, you must carefully review the subjects and subjects that you have already given to the Degree but which are fundamental to understanding the subject, especially the aspects of hormones, genital cycle, embryonic development, anatomy and function of the genital apparatus, ethical concepts in relation to human sexuality.

CM: The master class is the scene in which a teacher transmits the knowledge in a classroom to the whole group of students. The format, however, allows the introduction of activities in small groups within the classroom and the development of strategies that encourage the active participation of students.

MC: The method of the case consists in the process of solving clinical or professional cases of the profession. They are group activities that are solved with the active participation of the teacher after the deliberation of the students.

ABP: Problem based learning has similarities with the case method but differs in that students are responsible for finding the solution to the problem. The teacher is a tutor who facilitates the learning process based on his metacognitive skills. It is also done in groups.

LH: The skills lab is carried out in small groups, it is an area where the student can basically develop communication and physical exploration skills based on models, robots, simulating patients or real patients. It is also an area for the most practical knowledge of complementary explorations and therapeutic measures.

Evaluation systems and criteria

In person



Evaluation criteria: 

LH: compulsory attendance 10%
MC and ABP: NO compulsory but recommendable attendance and the participation and punctuality will be valued. It will include an evaluative minitest.
The exams will include questions from both CMs and MCs and ABPs.
The level to approve is 5.00. 4 MH can be given. There will be no exam to improve grades. Only in the case that the number of students exceeding 9 is greater than the number of MH, will a test be made for its award.

- Partial Exam (20%): test type test on theoretical issues, images and resolution of clinical cases. It does not release agenda.
     - The questions have 4 possible answers. 30 questions.
     - The partial exam is on all the subject taught to date.
- Final exam (55%): test type test on theoretical issues, images and resolution of clinical cases.
     - The questions have 4 possible answers. 95 questions.
     - The final exam is about all the subject of the course.

In the test type test will be scored with 1 point each correct answer. For each question with incorrect answer, one third of the value of the question will be subtracted.
Public discussion will be held on the questions of each exam.
The individual review will be made upon request.

Evaluation of 2nd convocation:
   - The examination of 2nd call is about all the subject of the course.
   - The value of the exam will be 90%.

 

LH: COMPULSORY attendance

MC and ABP: COMPULSORY attendance:

  1. A presentation of the topic by the teacher will be provided (document in Moodle). 
  2. Clinical cases or problems to be developed by students in a group.
  3. Presentation by the diferent students, with the corresponding feedback from the teacher.
  4. Short questionnaire at the end of the CM/PBL.

Theoretical evaluation:

  • Partial exam (20%):
  1. You ask them they have 4 possible answers. 30 ask.
  2. The partial exam is about all the subject matter imparted to the data.
  • Final exam (55%):
  1. Multiple choice questions of reasoning, based on clinical cases, memory, etc.
  2. The time will be adjusted per question.
  3. If the exam is carried out in telematic mode, computer tools will be applied that limit the possibility of consulting notes.
