Pre-course requirements

Students will be familiar, prior to the seminar, with Part 14 (Surgery for implant installation) and Part 15 (Reconstructive ridge therapy) of the latest edition of Clinical Periodontology and Implant Dentistry (Blackwell-Munkgaard. 5th Edition).

Objectives

To review the surgical aspects of implant dentistry and apply its fundamentals to a hands-on model.

Learning outcomes

This seminar will be mandatory for all second and third year students. Students will be evaluated after completing each step of the hands-on seminar; a final qualification of 0-20 will be given depending on their clinical performance, interest and participation. Students will be evaluated on their theoretical knowledge. After the clinical part of the seminar has concluded, students will take a written test based on 20 questions concerning the topics covered during the seminar. The final degree of the seminar will be based on the sum of clinical and theoretical qualifications divided by 4. This will give a 0-10 based degree.

Syllabus

Instrumentation: The instruments needed for the hands-on seminar will be reviewed. The applications and use of each of them will be described thoroughly.

Incision design and flap elevation: The biology of wound healing will be discussed in brief.  The different incision designs for ridge augmentation and implant placement described in the literature will be discussed. A crestal incision with a mesial vertical releasing incision will be performed in the edentulous area mesial to the canine of the pig jaw.

Implant placement (healed ridge and extraction socket): The fundamentals of prosthodontically driven implant placement will be reviewed.  Two dental implants will be placed in each side of the pig jaw. One in the edentulous area of the pig jaw distal to the canines and the other one in an extraction socket, after the extraction of the first the premolar. Each student will work on one side of the jaw.

Bone regenerative procedures: After implant placement in the edentulous area has been completed the implant will be removed from the osteotomy and a 5mm dehiscence will be created on the buccal aspect of the osteotomy. The implant replica will be inserted again on the edentulous ridges. The bone graft substitute will be applied over the dehiscence defects. A resorbable collagen membrane will then be trimmed and applied to cover the peri-implant defect. The fundamentals of periosteal releasing incisions and passive wound closure will be explained in detail.

Suturing: The different suture materials available will be reviewed. Suturing techniques will be described. The use of mattress sutures to attain passive closure over regenerative sites will be emphasized. The theoretical aspects will be applied on the pig jaw.

Model: Pig jaw mandible. Students will work in pairs. Each pairs will use one pig jaw.

Materials: Surgical Kit, implant replicas and implant motor. Resorbable collagen membranes, bone graft substitute.
